Request a Mayoral Proclamation

Mayoral proclamations are provided to individuals and organizations who have made outstanding contributions to civic life in the City of New Haven. If you believe an individual or organization should be acknowledged in this way, please fill out the form below to the best of your ability for mayoral review and consideration. Respectfully, proclamation requests must be made at least two weeks in advance, and a month in advance if you are requesting a large volume of proclamations at once. Please provide as much biographical information as possible--suggested information: 1) Schooling – where did they go to high school and/or college and/or other degrees? 2) Career – where did they work and/or what did they do and/or were they a full-time parent? 3) Family – How many children did they have, who was their spouse, etc.? 4) Community Service – have they been involved in any volunteer or community service or organizations? 5) Residence: a. Where were they born and grow up? If not New Haven, when did they move here? b. How long have they lived in New Haven? c. Why do they live in New Haven or love living in New Haven? 6) Anything else notable?
